Portfolio Forms

As a member of the STRC, your experience living in Woldt Hall is not only largely based on your academic achievement, but also your involvement in the STRC community as well as the broader CMU community.

The following forms are required to be turned in at various times during the Fall and Spring Semesters. Please check this list often to ensure that you are filling out (and turning in) the most recent, updated form for your records. Remember, these forms are your responsibility, and if you lose a printed copy, extras are always available here on this Web site.

Participation Records and Academic Event Participation Form

If you want to receive credit for participating in an academic, social or service event, you will need to have the appropriate form with you at the time of the event. For social and service events (or programs), all you need is your current Participation Record (see list of Participation Records below).

When you attend an academic program or event, you need to have an Academic Event Participation Form with you. Unsure about what program or event qualifies? Consult the list of Activity and Event Categories. If you have questions regarding an unlisted activity or event, please contact the STRC Academic Director.

Academic Advising Form

Each semester you will need to meet with your CST faculty advisor regarding courses you should take and your academic progress. It is important that you do this in a timely manner; make sure to schedule an appointment with your advisor well before the advising form is due.

Remember, if you do not have your advising form turned in on time, you will not be eligible for pre-registration!
